We excel at care and are proud of it. Resident care is led by our Director of Health and Wellness who determines care needs and designs an individualized service plan for each resident. This essential leader also participates in the training and supervision of our caregivers, which is absolutely fundamental to our Higher Standard of Care.

We also bring a very proactive approach to the support of our residents’ families, with frequent family and Care Connection meetings, typically more than required by the states in which we operate. There we can listen to feedback about our service from the resident and their family, and together establish new goals for the months ahead. Care Connection meetings are attended by our Executive Director and Director of Health and Wellness as well as any other team members that may be involved in the resident’s care and, of course, the resident and a family member. We focus on things like falls, weight loss, skin issues and behaviors that may be keeping you up at night as a family member wondering about your loved one’s overall health and well-being. We have many evidence-based approaches and best practices to identify these issues, intervening early to reduce the likelihood of negative outcomes. It’s a complete dedication to excellence in your loved one’s overall experience.

We also embrace and will assist with coordinating service with other providers. Our clinical team works closely with other providers to make sure care is seamless and to lift the burden from you. Of course, you are still involved and as engaged as you want to be.

Prior to arrival, upon move in and regularly thereafter, your loved one’s care needs and medications are evaluated by our nurse and inform a comprehensive, personalized plan of care that is delivered by our specially trained caregiver.
